No Show, Cancellation, Financial Policy
Thank you for choosing KJ Relaxin Medical Spa for your Medical Spa needs. We are committed to your treatment being successful. Please understand that payment for your services is considered a part of your treatment, and we require payment in full prior to your first treatment.
We accept cash, checks, Traveler's Checks, Mastercard, Visa, and Care Credit. Please let us know if you have any questions or concerns about this policy or your treatment.
Full payment is due prior to your first treatment.
Competitive Services
Our practice is committed to providing the most competitive prices in the area. If you provide proof of a comparative medical service offered for a lower cost, we will match that price and give you an additional 10% off.
Please note that we provide a medical level of service when making price comparisons.

Medical Information
By completing a Confidential Medical History, I understand that I have provided KJ Relaxin Medical Spa with all the details of my medical condition(s).
I understand it is important for the Aestheticians to know this information prior to any treatments.
It is also my responsibility to inform KJ Relaxin Medical Spa of any changes to my medical condition.
Staying on Schedule
I understand that it is important to stay on schedule for my treatments to be effective.
Moving my appointment out a week can dramatically change my results.
Returned Checks
I understand that if my check is returned to KJ Relaxin Medical Spa for any reason, I will be charged a $30 fee.
KJ Relaxin Medical Spa will notify me if this occurs.
I agree to pay the original amount plus the additional $30 within 10 days from the date KJ Relaxin Medical Spa notifies me of the returned check or prior to my next appointment, whichever occurs first.
Balances
I understand that any outstanding balance that I have with KJ Relaxin Medical Spa is due prior to my next appointment.
Cancellations
When a cancellation is necessary, please call at least 24 hours in advance.
Wedding parties and Spa parties require a 14-day cancellation notice.
A credit card will be required to reserve all packages, bridal parties, and when booking any service of $100 or more.
If you cancel an appointment after 24 hours and are a no-show, we reserve the right to charge your credit card for the full amount of services booked.
After 2 missed appointments or no-shows, we will require a credit card to re-book any new appointments.
